Hmmm, subsequent requests by FF result in the double hit.

Mark, have you considered that maybe this is once for HEAD request to
see if the image changed since the last request, and once for GET when
nothing definitive comes back from HEAD?

If you add $_SERVER['REQUEST_METHOD'] to the output, we might solve this.
